IMPORTANT:If you hold an active U.S. state/territory license or have five or more years of ophthalmic dispensing experience from an unlicensed state/territory, you are eligible to use this Special Application to apply for a Nevada Ophthalmic Dispenser License. You will not be able to save your application to return to later. Please read the following requirements carefully and ensure you have digital copies (PDFs or clear photo files) of all required documents before proceeding.

Required Documents (Uploads)
Government-issued identification. Acceptable documents include a copy of a U.S. birth certificate, U.S. passport, U.S. Social Security card, NV Real ID card (with star icon at top right), U.S. Worker’s Visa, U.S. Permanent Resident Card, or U.S. Certificate of Naturalization.
Proof of secondary school completion. Acceptable documents include a copy of a high school diploma/transcript or GED certificate. If you received your secondary school education outside of the U.S., you must include translated and verified copies of any transcripts or certificates.
Certificates for any ophthalmic education/training programs you have completed. You must upload copies of your certificates and have your schools mail official transcripts directly to the board office for any completed programs.
Ophthalmic dispensing license(s)/certification(s) issued by other states/territories/jurisdictions. You must upload copies of each license certificate and have each licensing/certifying body send a License Verification Form directly to the board office. If you do not hold an active license, you must upload a complete resumé detailing your ophthalmic dispensing experience, including dates, locations, and employer contact information.
American Board of Opticianry (ABO) Advanced and Practical Certifications (must be current). If you do not hold the Advanced and/or Practical Certifications, please upload a copy of your Basic Certification and the board will notify the ABO of your eligibility to register for the required exams. You will not be eligible to register until you obtain the Basic ABO Certification.
National Contact Lens Examiners (NCLE) Basic, Advanced, and Practical Certifications (must be current). If you do not hold the Advanced and/or Practical Certifications, please upload a copy of your Basic Certification and the board will notify the NCLE of your eligibility to register for the required exams. You will not be eligible to register for the exams until you obtain the Basic NCLE Certification.
Contact lens training verification (minimum 100 hours). Verification may be a letter from the person(s) who provided your training detailing dates of training and specific topics covered, or official transcripts/certificates of training from accredited educational institution(s). Letters must be emailed directly to the board office at info@nvopticians.org and transcripts must be mailed directly by the schools.
Three reference letters. At least one letter must be from a person who has worked in the optical field with you. Letters must be emailed directly to the board office at info@nvopticians.org.
Laws and Ethics Exam Requirement: Once your application is complete, including all required certifications, the Board will issue a Laws and Ethics Exams by mail.This “open book” exam covers the Laws and Regulations that govern a Nevada license.
